Phototherapy Phototherapy using narrowband or broadband UVB or, less commonly, UVA regimens including psoralen plus UVA (PUVA) and UVA1, may be used for patients unresponsive to standard therapy. 42,43 The addition of topical corticosteroids to UVA/UVB therapy may allow reduction of the UVB dose and duration of treatment. 44 Unfortunately, there is frequently a relapse following the cessation of phototherapy. Access to treatment centers can be difficult for some patients, and there are concerns about adverse side effects such as premature aging of the skin and cutaneous malignancies. Systemic Cor ticosteroids Oral glucocorticoids may be beneficial for severe cases to abort major exacerbations, but are generally avoided and used only in short courses of a few days to a week. Use is limited by the frequent rebound flares occurring at discontinuation. In addition, the numerous systemic side effects including adrenal insufficiency, osteoporosis, immunosuppression, pseudotumor cerebri, pancreatitis, hyperglycemia, hypertension, electrolyte abnormalities, psychosis, and cushingoid habitus makes it impractical for recurrent or long-term treatment.
